To receive any records from the vitals section you must provide proper identification, correct payment for fees, and fill out the required request form which can be obtained above.
For birth or death certificates, you must be either the owner of the document or related as legal guardian, mother, father, sister, brother, grandparent, son, daughter or current spouse.
